Gallant Care offers home care services, including but not limited to:​

• Personal care assistance.​

• Medication management.​

• Companionship services.​

Client Responsibilities

• Provide accurate health and personal information.​

• Notify us of any changes in condition or care needs.​

• Treat our staff with respect and dignity.​

Billing and Payment

• Services are billed [weekly/monthly], with payment due [upon receipt/within X days].​

• Late payments may incur additional fees.​

Termination of Services

Either party may terminate services with [X days] notice.​

Limitation of Liability

Gallant Care is not liable for any indirect, incidental, or consequential damages arising from our services.​
